2. Have you traveled internationally within the last 14 days? 您是否在最近的14天内有过国际旅行？

3. Have you had unprotected close contact with individuals who have a confirmed or presumptive diagnosis of COVID-19?您是否接在无防护措施情况下密切触过确诊或疑似新冠肺炎病人？

 

If you answered “yes” to any of the questions must not make an appointment for an in-person treatment. We advised you should self-isolate and call Health Link 811. 如果针对上述任一问题，您回答为“是”，请勿预约上门就诊，请拨打811健康热线详细咨询。

If you answered "no" to all of the questions, you can book online. 如果您对所有问题均回答“否”，则可以继续在线预约。
